Why claim denials cost more than you think

Industry data consistently shows that between 5% and 10% of medical claims are denied on first submission, and a large share of those are never reworked. Every denied claim costs staff time to investigate, correct, and resubmit, and each day it sits unpaid stretches your accounts receivable. For a mid-size practice, denials can quietly erase tens of thousands of dollars a year.

The good news is that most denials are preventable. They tend to cluster around a handful of root causes: eligibility problems, missing authorizations, coding errors, incomplete documentation, and timely filing misses. Fixing those five areas resolves the majority of denials for most practices.

1. Verify eligibility before every visit

Eligibility and coverage issues are the single largest denial category. Verifying insurance in real time before the patient arrives catches terminated policies, changed plans, and coordination-of-benefits problems while there is still time to collect correct information from the patient.

  • Run batch eligibility checks 48 to 72 hours before scheduled appointments

  • Re-verify at check-in for walk-ins and same-day visits

  • Confirm copays, deductibles, and referral requirements, not just active status

2. Track prior authorizations centrally

Missing or expired authorizations lead to hard denials that are difficult to overturn. Keep a single authorization log with the payer, the approved service, the number of visits or units, and the expiration date. Assign ownership so that nothing falls through the cracks when staff are out.

3. Invest in coding accuracy

Coding errors, unbundling, and incorrect modifiers trigger both denials and audit risk. Certified coders who review documentation before claims go out catch problems that automated scrubbers miss. Regular coding audits also surface documentation gaps that providers can fix going forward.

4. Scrub every claim before submission

A pre-submission claim scrubber checks for demographic mismatches, missing NPI numbers, invalid code combinations, and payer-specific formatting rules. Clean claims are paid faster and rarely bounce. Practices that scrub every claim routinely reach first-pass acceptance rates above 98%.

5. Analyze denial trends monthly

Denials are data. Categorize every denial by reason code, payer, provider, and service line. Within a few months you will see patterns, such as one payer rejecting a particular modifier or one location consistently missing authorizations. Address the root cause and the denial category disappears.

Simplify Billing Services applies all five of these practices for every client, which is how we maintain a 99.9% claim acceptance rate. If your practice is fighting denials, contact us for a free denial analysis.
